Dr. Simon Robson, MD: Diabetes, obesity and nonalcoholic fatty liver disease is a major epidemic that we are going to see. Nonalcoholic fatty liver disease can progress to inflammation. We then call it steatohepatitis.

Steatohepatitis simply means "liver inflammation in the presence of fat". Many patients with steatohepatitis can worsen. Patients can suddenly present with liver cirrhosis.

Patients with nonalcoholic fatty liver disease and steatohepatitis also can first present with complications of cirrhosis. They can have liver cancer.

Patients are predisposed to nonalcoholic fatty liver disease and steatohepatitis. They are usually morbidly obese. They often have diabetes.

Such patients are often treated with medications to treat Type 2 diabetes. Patients take oral hypoglycemic medications. There is not much we can do to preclude the development of nonalcoholic fatty liver disease (NAFLD) and steatohepatitis.

The best prevention is to address the fundamental metabolic problems that lead to NAFLD and steatohepatitis.

Other major form of liver disease is autoimmune liver disease. It can present as liver manifestation of rheumatic disease (rheumatological liver disorder). Autoimmune liver disease can also present as inflammatory bowel disease or celiac disease.

Early diagnosis of autoimmune liver disease is important. Because we could miss correct diagnosis. We could fail to treat the patient with immunosuppressive medications.

Patients can progress to liver cirrhosis, liver failure and death. Liver transplantation is the only treatment at advanced stage of autoimmune liver disease.

We still see liver inflammation associated with metabolic disorders. Alpha-1 antitrypsin deficiency is one example of liver inflammation disorder.

We also see liver problems associated with iron storage (hemochromatosis) and copper storage (Wilson's disease). We also see liver diseases that are caused by problems with glycogen storage (glycogen storage diseases). Glycogen is animal starch.

Iron storage disease, Wilson disease, and glycogen storage diseases are much more rare. Nonalcoholic fatty liver disease and steatohepatitis are more common.

I have spoken of viral hepatitis B and hepatitis C. Other forms of liver disease are increasing in frequency. Because we can now treat more effectively hepatitis C and hepatitis B.

Nonalcoholic fatty liver disease, steatohepatitis and diabetes are increasing. Diabetes is also rising. Alcoholic liver disease is a problem in North America and Europe.

Dr. Anton Titov, MD: Rates of alcoholism and alcohol usage are increasing dramatically. There are few effective therapies for alcoholism.

Dr. Simon Robson, MD: There are minimal antidotes to prevent liver injury if patients continue to drink alcohol heavily. We will discuss alcohol liver disease later in the interview.

We will discuss nutrition, toxins and liver disease. It is important that we immunize patients against Hepatitis B and Hepatitis A. We have vaccines against Hepatitis B and Hepatitis A.

It is important to diagnose other liver diseases early, such as iron overload (hemochromatosis). Early diagnosis of copper storage disorders (Wilson's disease, Wilson disease) is important.

It is important to make early diagnosis of autoimmune liver disease associated with inflammatory bowel disease. Not only viruses cause hepatitis.

Many other diseases cause liver inflammation. Many other disorders can damage the liver.
